Public Health Institute Features ACCA Care Navigators

Public Health Institute (PHI) recently published an impact article featuring AC Care Alliance’s Care Navigator model. In the report, PHI describes how ACCA Care Navigators work with participants through the Advanced Illness Care Program providing trusted referrals, resources, and tools to empower them to achieve their identified goals within the Five Cornerstones of spiritual, health, social, advance care planning, and caregiving needs. The report goes on to show how coming from and being embedded in the communities they serve allows Care Navigators to build relationships and trust among participants and the community.

AC CARE Alliance’s model is also building professional pathways for individuals from the communities they serve, helping to support a stronger public health workforce in California that reflects the state’s diverse communities, and is better equipped to meet the world’s pressing health challenges and address inequities.
