AC Care Alliance Holistic Approach to Care
Enhanced Care Management (ECM) is a holistic, patient-centered approach to care. You are more than just a diagnosis and ACCA seeks to address your need in three pillar areas that include your physical health, mental health, and social health. In addition, ACCA will help bridge health care gaps, advance equity, and address health disparities to make sure you get the care you deserve from the health care system.
ECM is a FREE benefit for qualifying Medi-Cal beneficiaries through California’s Department of Health Care Services’ (DHCS) CalAIM Program. Enrolled participants will have the support of a multi-disciplinary team including a trained Care Navigator who will walk alongside you both in-person and over the phone. At the end of the year, it is our desire that you leave the program empowered to be better able to manage your own health and wellness and access the support you need.

Anthem Blue Cross members in San Francisco and Santa Clara counties
Kaiser Permanente members in Los Angeles county
What Can ECM Help Me With?

After a comprehensive assessment, receive personalized goals that includes your priorities and actionable steps to reach larger goals.

Get help making and preparing for appointments with your physician, specialists, and mental health providers to access the care you need. Ensure your questions and concerns are addressed and get follow up services after medical appointments.

Learn skills and strategies to better manage your health and medical conditions at home.

Receive personalized support to ensure that your wishes are documented and your voice is heard in the event you are unable to speak for yourself.

Receive trusted referrals and application assistance for social services such as food and transportation services, caregiver support, and housing.
