Center to Advance Palliative Care Recognizes ACCA as one of the Most Replicable Interventions to Advance Health Equity

In 2021, the Center to Advance Palliative Care (CAPC) launched Project Equity, an ongoing initiative to address inequitable care for certain groups of people living with serious illness who have disproportionately experience poor care. CAPC conducted a comprehensive environmental scan to better understand the health care experiences of Black Americans living with serious illness, and their families, and how to bridge disparities in access to palliative care services. The scan sought to identify opportunities to reduce these inequities, and ultimately uncovered hundreds of interventions that are moving the needle on health equity. AC Care Alliance was highlighted in CAPC’s Innovation Hub as a replicable solution making a breakthrough change in the care of serious illness and was featured in a 2023 article entitled “Inspiring Teams to Engage in Health Equity Interventions to Improve Care for Black Patients and their Caregivers” by Brittany Chambers, MPH, MCHES and Chelsea Perez.

In the article, ACCA’s Care Navigation and Five Cornerstone model of the Advanced Illness Care Program (AICP) was featured with several key takeaways being highlighted: the importance of trust between faith-based organizations, community members, and health institutions, organizations ensuring the community they serve is represented in the clinicians and staff they employ, and utilizing storytelling and story listening to build trusting relationships, learn concerns, and tailer the AICP to meet their needs.

Trust between faith-based organizations, community members, and health institutions must exist for any such program to operate. Additionally, organizations must do their best to ensure the community they serve is represented in the clinicians and staff they employ.
