California Health Care Foundation Features AC Care Alliance

In an article entitled “Alliance with Black Churches Closes Care Gaps for Aging People," author Heather Stringer featured the work that AC Care Alliance Care Navigators are doing with persons needing care in the Bay Area and Los Angeles. In addition to providing an overview of the Advanced Illness Care Program and the partnerships that have been developed, she also highlighted the assistance Care Navigators provide navigating confusing medical and support systems and the relief that caregivers feel when working with the ACCA.

The mission of the California Health Care Foundation (CHCF) aligns strongly with the mission of AC Care Alliance as it seeks to bring health equity for all Californians, with a special focus on low income individuals and communities facing structural barriers to care such as the color of their skin, their ethnic background, or the language the speak.
