Churches:
Our churches have been a backbone of the work we do, enabling us to expand outreach not only to people within our churches, but to the broader community as well.
In 2013 the Alameda County Care Alliance was formed by 5 hub churches and today our network includes more than 40 churches across all our active counties and it continues to grow! In many of our churches, appointed champions help identify and refer congregation members for possible program enrollment.

UC Davis Betty Irene Moore School of Nursing
The Betty Irene Moore School of Nursing at UC Davis has been a key partner for many years, providing technical assistance, leading the program evaluation, collaborating with the team to refine the intervention, and participating in strategic planning.
Dr. Janice Bell has lead the program evaluation for the ACCA, Dr. Jill G. Joseph has been instrumental in helping to refine the AICP intervention, and Rebecca Badeaux has contributed her immense talent in developing numerous short videos for the ACCA.
Public Health Institute:
In 2017, ACCA became a program of the Public Health Institute (PHI) which served as ACCA’s fiscal sponsor until 2025.
PHI is a 501c3 organization dedicated to improving health, equity and wellness. As one of the largest and most comprehensive public health organizations in the nation, PHI has hundreds of programs with robust expertise in public and community health research and evaluation, chronic disease prevention and management, collaboration amongst diverse stakeholders and building capacity for strong public health policy, programs, systems and practices.
Health Systems and Health Plans:
ACCA collaborates with health systems, Federally Qualified Health Clinics, and community organizations.
ACCA has completed pilot programs and has collaborated with health organizations to establish referral pathways with major healthcare providers, health systems, and community health facilities. These health organizations include Kaiser Permanente in the Bay Area and Los Angeles, Lifelong Medical Care, Stanford, Ravenswood Family Health Network, Roots Community Health, Alameda Health System, Tiburcio Vasquez Health Center, Community Health Center Network, and numerous home health and acute health centers.
In 2025, ACCA expanded the AICP program to meet requirements of California’s Enhanced Care Management (ECM) and contracted with Anthem Blue Cross to become an ECM provider.
