Churches:

Our churches are the backbone of the work we do, enabling us to expand outreach not only to people within our churches, but to the broader community as well.

People dealing with advanced illnesses and their caregivers of all backgrounds should know we exist to serve you!  

In 2013 the Alameda County Care Alliance was formed by 5 hub churches and today our network includes more than 40 churches in Alameda County, Contra Costa County, and San Francisco, and it continues to grow!

In many of our churches, appointed champions help identify and refer congregation members to a designated Care Navigator for possible program enrollment.  We are grateful for the involvement and support of our partner churches.

Funders:

ACCA is fortunate to have been supported by funders who believed in us and in the value of our program. 

Our funders have made it possible for us to provide our Advanced Illness Care Program™ free of charge to more than 1400 individuals and reached more than 15,000 people with information about our services and the importance of advance care planning.

UC Davis Betty Irene Moore School of Nursing

The Betty Irene Moore School of Nursing at UC Davis has been a key partner for many years, providing technical assistance, leading the program evaluation, collaborating with the team to refine the intervention, and participating in strategic planning.
Dr. Janice Bell leads the program evaluation for the ACCA, Dr. Jill G. Joseph has been instrumental in helping to refine the AICP intervention, and Rebecca Badeaux has contributed her immense talent in developing numerous short videos for the ACCA.

Public Health Institute:

Since 2017, the ACCA has been administratively housed at the Public Health Institute (PHI) which serves as ACCA’s fiscal sponsor.

PHI is a 501c3 organization dedicated to improving health, equity and wellness for over 56 years. As one of the largest and most comprehensive public health organizations in the nation, PHI has hundreds of programs with robust expertise in public and community health research and evaluation, chronic disease prevention and management, collaboration amongst diverse stakeholders and building capacity for strong public health policy, programs, systems and practices.

Health Systems:

The ACCA collaborates with health systems, Federally Qualified Health Clinics, and community organizations.

Currently, the ACCA is fielding two pilot programs and collaborating with health organizations in the Bay Area after establishing referral pathways with another 10 organizations, including major healthcare providers in Northern California, a major urban health system, and multiple community health facilities. These health organizations include Kaiser Permanente Oakland, Alameda Health System, Lifelong, Community Health Center Network, DayBreak and the Felton Institute.
