The AC Care Alliance (ACCA) was recently highlighted by the California’s Health Report following the award of the Hillman Innovations in Care Program Grant from The Rita & Alex Hillman Foundation. In the article entitled “Caring for Advanced Illness Through Faith Communities” (2016) author Matt Perry acknowledges how faith-based communities continue to be an avenue through which community health experts connect with older adults and highlights ACCA’s model of having care navigators partner with African-American churches to help congregants.
Despite growing national attention to advanced illness care, African Americans experience consistent disparities including a disproportionate burden of advanced illness,” says Janice Bell of the UC Davis Betty Irene Moore School of Nursing, which is collaborating with the alliance to offer training and outreach. “(This program meets) the advanced illness care needs of African American community members in ways that align with their spiritual values and cultural preferences.
California Health Report was founded in 2009 and is an independent news outlet whose mission “is to report from communities that are disproportionately affected by inequality.” They partner with communities of color and report on health equity issues and how they impact California’s most vulnerable.
