ACCA Brings Advance Care Planning to SF 8/11/2021
ACCA is a Learning Journeys Community Grant Recipient
On 8/11, thanks to a special Learning Journeys Community Grant, the ACCA held the
“Choose to Prepare – The Power of Advance Care Planning” event for members of the San Francisco community.
This socially distanced in-person event was also live-streamed for virtual attendees and featured a powerful personal testimony by Crystal Walker around the importance of putting your wishes in writing for a time in your life when you may not be able to speak for yourself. Ms. Walker, a well-known advocate and valuable asset to the Bayview community, is a compassionate woman with a strong desire to empower underserved communities with advanced care planning.
At the event, attendees had the opportunity to learn about the ACCA’s Advanced Illness Care Program, a holistic approach to connecting individuals with trusted resources and providing empowerment through support within our 5 Cornerstones. With advance care planning the focus of one of our Cornerstones, attendees were able to sign-up to work with one of our Care Navigators to receive our free services. Each person received a copy of the ACCA’s Care Planning Workbook, a gift card and a boxed lunch.
